Yearly payslip Summary

I am trying to create a yearly payslip summary for an employee, but cannot
find a template any-where. All that is needed is what he has been paid in
the past 12 months, set out in a comprehensive form. Can any-one help me?

When you say "comprehensive form" are implying a printed report or a Form
object?
You can create a query with the criteria (I am assuming that you have a date
field associated with his/her P/R): Between [date 1] and [date 2]. If you are
creating a report just make sure to include the Sum in the [Summary] option
for your currency fields.
