The big question is: Are there signed drivers available for all of your
hardware devices? You will need these for it to work. Much may be natively
supported by Vista, but the manufacturer is the best source of information
and required drivers. Check here for additional information on whether a
component is compatible:

Is it worth it? Well, that depends. Do you have applications that can take
advantage of 64-bit processing? More than 4GB of system memory? If so, then
posssibly yes, otherwise your programs will run in a 32-bit mode that is no
faster than the x86 version.
Upgrade? No, it'll have to be a clean install. You can use the Easy Transfer
and ET Companion to help in the migration:

Opinion only: Make an image backup of your current system. Then go ahead and
install x64 and see if you can get everything working the way you want it
to. If so, great. Have at it and enjoy (I do). If not, restore the image,
many bits and pieces of computing in the home are simply not ready for x64.
