Word locked Form help!!

I'm finishing off a report form for work. Everything is finished and I'm
happy with it apart from 1 thing.
At the top of the form I have name, address, date, call number. On the
subsequent pages I have a header with date and call number. I want the user
to enter the date and call number once from the top of page 1 and repeat the
data in the header information.
Any ideas? i don't understand macros or javascript but any help would be
great.
