I need to attachment a preformatted document to a *.doc.

I have done the following procedures,

1, "Insert"
2, "Picture"
3. Scan from scanner.

This works fine as a *,doc attachment but I need original size of the scanned document. Is there an option to do this?

it's never easy to combine two documents that were not created from the
same template (_and_ where the creators used the same styles for the
same things, and next to no direct formatting :)). In your case, the
scanned document will not conform to any existing styleguide, so the
problem is really big.

Your best bet is to convert both documents to PDF and combine the result.

Otherwise, you will have to do a lot of reformatting, either through
VBA, or manually.

If you insert an image into a Word document, it will adapt to the current
margins. If you change the layout property to something other than in-line
you can drag the image beyond the margins to fill the page, or set the size
property to 100%
