Windows overlay or repair installation

What exaclty occurs when you do an overlay of the operating system? What
files are deleted/restored? Is the registry tampered with at all?

Reason I ask is I'm having a big issue with Windows Update and there is no
solution to the problem. The only solution is to do a repair installation
of the OS.

Perhaps if I can figure out what exactly happens when you do an overlay I
can maybe pinpoint the steps one by one and see if I can find a fix for this
issue without having to reinstall Windows.

The system files are replaced by the versions on the installation media. Any
ones that have been updated via ServicePack or other update installation are
undone. The registry is reset to system supporting defaults, but entries
placed by other programs are not affected. Other than system files, nothing
is deleted or restored.
