Why is header and footer insert autotext grayed out?

When I open a new document the cursor is always at the very top of the page,
no margin shows. This is not how it was when I first installed it, if you had
a margin of 1" the cursor showed 1" down from the top of the page. Once I
open a new document and then view headers and footers the margin comes back,
although the option of inseting autotext in the header bar is grayed out and
I can no longer select an automated function, like insert page of page. Any
help would be greatly appreciated.

The autotext function on the header toolbar requires that there is an
autotext entry actually available in the paragraph style of the
header/footer. Any other failings of commands may infer a more serious
issue like a corrupt normal.dot template (rename it) or a minor registry
