Why do Word documents save only as Wordpad and lose formatting?

When I try to save a Word document it always becomes Wordpad. I have checked
Tools/options/save and it is set to save as Word.
 
It's still a Word doc; it just has a WordPad icon because the file extension
has become associated with WordPad. Here are a couple of ways to fix that:

1. Reregister Word; this has nothing to do with registering the program with
MS but with causing it to reestablish its ownership of the .doc, .dot, and
other file types in the Windows Registry. At the Start | Run prompt, type

winword /r

and press Enter. Note that there is a space before the forward slash. It may
appear that nothing is happening, or you may get a message box. Word will
not open, but your file associations should be repaired. For more, see

2. In Windows Explorer, select one of the files, right-click, choose Open
With, and select Microsoft Word. Check the box for "Always use this
application to open files of this type" (or whatever the exact wording is).
