All doctors, and most insurance companies, have paper forms that they use to
gather patient history, I would suggest those as a starting point for
developing a patient system. Be careful. Current HIPPA law requires patient
privacy. Access, being a desktop database is difficult to secure fro medical
purposes. Make sure that you follow all the security guidelines outlined in
the security FAQ:

i fully understand what you mean..but i am a medical student and what i need
is for a research project of mine..i want to have such a database for the
patients i observe so and i can categorize them with full hospital
information...the private information is not so much neccessary unless i find
some critical points about them that need to contact them urgent..we can
secure all these info. we just need to have such a program and i think it is
more secure than papers on file cabinets..
