where can I find a blank telephone index to download?

I would like to find a place to display my own telephone directory, -
somewhere I could find quickly. just names and telephone numbers. Any
suggestions?

There are several templates that come with Access. Just go to New. Then
select the second tab in the window and find one that works for you. Most
of them go well beyond what you want to do.

If it just a list of names and numbers, a Excel spreadsheet would be
the easiest. Depends on how many numbers you have.
