WHAT CAUSES BUG IN OUTLOOK CUSTOM FORMS?

Hi,
I have designed a custom form in Microsoft Outlook 2003, Build 11.5608.
When I send the custom form, occasionally the recipients cannot open the
form. (Recipent must delete
all FRMcache.dat files for form to load properly)

Does anyone know what triggers this and/or how to prevent it?

Office 2003 Service Pack 1 cures most of the forms cache corruption problems
in the original release.
