Wanting to create a dynamic form

I am trying to design a dynamic form. I want the form to be able to connect
with several tables, roughly 15 or 16. The idea is for the user to be able to
pick which specific table the data will be pulled from and put into. I am
looking to set up 6 different forms in this matter, each handling their own
set of tables. If it helps, this project is for a Water Pumping Division.
Please help me.
 
Doing this suggests that the multiple tables a form might be based on have
the same fields. If that's the case, then my question is why do you have
multiple table with the same fields?
