I am assuming you have an active resident virus scanner
running which is telling you that an infected file is
being accessed when you start windows. Though this file
is being cleaned, there still might be another file (the
original possibly) that is infected on your hard drive
that keeps spreading it again.
If you haven't tried it yet, be sure to run a full virus
scan on your computer with your anti-virus software. Your
scan should then remove everything.
You can also try an online virus scanner. Here's a good
one to try:

How can I determine what service or application is
loading a virus at every startup?
Norton AV catches a hookerdll.dll in the Windows
directory and deletes it but it doesn't stop it from
coming back at startup.
