View Header and Footer Dialog box

Word 2003 on Windows XP. I have a good dialog box that pops up when I
navigate the top menu via

\View\Header and Footer

The problems:

1. From a mistake I made that I can't replicate/don't remember, I now have
two (2) "Insert AutoText" bars on this box and can't seem to remove any of
them

2. There are times when my most-used command of "Filename and path" does
not appear as an option and there's no reason why I can think that this would
happen.

Anyone know enough to suggest the fix?

Possibly you've saved a document or template as Normal.dot?

Think I get that.....would mean that if I located and found this doc and
noticed that it behaved the same way that I should try to make changes there
that would then affect all docs made after.....

Is that correct?
