Using O/L Data to Populate Fax Cover Page (MS Word)

I would like to be able to use O/L Contact data to populate the name,
company and fax # fields of a standard MS Word Fax Cover Page Template.

I'm using OL 2000 SP-3 (9.0.0.6627).

Any refs on how to set this up?

Not a native feature. Most people just create a merge document in Word.
