Using a Survey Form

I'm using a Survey form made by Scott Bradley to create a survey that will be
sent to a number of people within my company. I enter all the questions and
then send it, but when the recipient tries to take the survey there are no
questions listed and it only resiters the existence of one question. The
form is there but no content....any suggestions?
 
That form, like most survey forms, works only if you publish it to the Organizational Forms library on your organization's Exchange server or to each user's Personal Forms library.
