Using a form and auto-populating fields

I've created a form in Word. I have one data field (name) that I need to
show up at the top of each page. Is there a way for the user to enter the
data once and then automatically populate the other identical fields?

For a protected form, the best way to handle this is with a StyleRef field.

See the article Suzanne referred you to. The idea would be to have the
single field for the first page. In the header of all other pages, you would
use a StyleRef field to display the contents of the first field. It isn't
intuitive but it works very well.
