user account locked

we have a user in active directory, when ever the account is logged out for any reason and the user goes to log back in the account is locked. any suggestions on how to correct this problem...

If the particular user have changed password and the account are used within
some services or application this may result in an account lockout if the
particular service or application use an old password.
