Unremovable Window/White Box on Desktop

When I boot up, a small white window or box about an inch and a half square
appears in the top left hand corner of the desktop overlaying the icons that
are there. It can't be moved or closed, but contains a list of times:

15 Minutes
30 Minutes
1 Hour
3 Hours
6 Hours
12 Hours
24 Hours

I can select the times (which colour that line blue) but nothing else
happens with a double or right click

What is it and how do I get rid of it?

The first thing to do is open the Display applet in Control Panel and look
on the Desktop tab. Click on Customize Desktop, and then click on the Web
tab. You will see that there are checkmarks next to "My Current Home Page"
and probably "Lock Desktop Items". Uncheck these, Apply/OK out. If that
doesn't work, you have something running in the background.

The First Question Of Troubleshooting: what changed between the time things
worked and the time they didn't?

The Second Question of Windows Troubleshooting: what is the malware/virus
status of the machine? If you think it is clean, what programs (and
versions) did you use to determine this?

In addition to Malke's post, does this happen if you boot into safe mode?
Restart the computer, as the BIOS splash screen disappears repeatedly tap
the F8 key to get the Advanced Options menu. Highlight Safe Mode, press
enter. Login to the account that has this problem. If it does not occur is
safe mode then reboot into normal mode and do some clean boot
troubleshooting to see what is loading at startup to cause this.
