The TOC could be including the style used on that particular paragraph, or
the TOC might be created with TC fields. Press Alt+F9 to show field codes,
and then tell us the field code for your TOC.

include only paragraphs whose styles have their outline levels set to 1, 2,
3, or 4 you would want the code to read: { TOC \o "1-4" \h \z }.

It isn't completely obvious from the field code what could be causing the
problem, but I'd recommend that you delete the \u switch. Then hide field
codes (for example by pressing Alt+F9 again) and update the TOC.
Also, which style is applied to the "problem paragraph"? You may want to
(re-)apply the Normal style, which can be done by pressing Ctrl+Shift+N.
