Trying to create an upload form to upload pictures

I am trying to create an upload form so users can upload pictures. I would
like the form contents and the pictures to go to the same place. When I try
to create a new folder as stated in the help. I don’t have an option to allow
scripts to be run or allow anonymous upload to this directory. These options
are light gray and I can not check the boxes

You have to check with your host and see if they allow you to use this component, and the server
must have the FP2002 extensions installed, and it has to be a Windows IIS server, and you must
publish to the server via FP's http mode.

You can only set/change permissions on folder on the live site via FP, and then only if the host
allows you to.

I have done everything you mentioned before but when I go to submit the file
it prompts me for a username and pass.
