Toolbars and addressbar keep reverting to default positions

Every time I open Internet Explorer my toolbars, links and addressbar go
back to their default positions. There was a patch that I installed that
kept this from happening. I had to reformat my HD and lost it.

WinXP Pro
Internet Explorer 6 sp1
