Tombstones in survey

I'm wondering how I can create tombstones in a survey using Word (2003). The
survey needs to be completed with pen and paper. So far the best I've come up
with is making a one row table and not making a border on the top of the
cells.

Any help would be greatly appreciated,

Tables with selective borders are often the easiest way to create
rectilinear graphics of all sorts including the boxes you want. I would have
thought the Box Drawing character subset (in Insert Symbol) might have what
you need, but there doesn't seem to be a bottom bar (like an underline). You
can also get crude ones with the ordinary pipe and underline characters.
