title missing

the title in the title bar is missing. The menu and an icon indicating which
program the window relates to shows. For instance, when I click internet
explorer, I get the blue color border, internet explorer icon in the upper
left hand corner and in the right corner I see the minimize, maximize, and
close symbols. Please help!

This is usually caused by Google Toolbar or Google Desktop search.
