the instruction at referenced memory at the memory could not be read

When I start my laptop and sometimes when I am using it
I get the following message:
the instruction at referenced memory atxxxxxxx the memory
could not be read.it is the same memory address each time.
I have checked the memory out and all is fine I simply
hit cancel to debug and i log in ok.
What is causing this to happen?
