I'm sorry for your need to create a note for this occasion, however this
group doesn't deal with content.

That having been said, just yesterday I was helping a coworker with phrasing
for a thank you card for flowers she received while she was out on a medical
leave. I can tell you that if you use Google to search, you can find MANY
sites with appropriate phrasing.
