text & checkboxes in document

I have created a document which requires the need for both checkboxes and
text input. I have tied locking the document and leaving sections available
for editing (no checkboxes in editable field) which works until I save and
reopen the document. If I copy the entire document (after it has been
reopened) and paste it on a new page it suddenly works again.

Any tips or ideas?

It seems as if the document is opening in design mode. (If you are
also seeing the Control Toolbox toolbar, that would be another
indication.) This is because of macro security settings.

Rather than lowering the security settings, it would be easier to use
a protected Word form. See
http://word.mvps.org/faqs/customization/FillinTheBlanks.htm.
