Task bar toolbars

I have a modified toolbar button I added to my task bar with commonly used
icons. For some reason it becomes unresponsive. When I delete it and try to
re add it, I get a 'Cannot create toolbar' error box. Does any one know why
this happens? There seems to be no particular pattern of when this happens.
