We have created a switchboard but it does not appear as a
form in the forms window. How can we get it to appear?
 
I know this is going to sound simple, but it is the first thing I ask when
this question comes up:
Does the Forms window have scroll bars on it? Are you sure your form isn't
just scrolled off the screen?
