Switchboard forms coming up blank

I have a switchboard I made using the Access Database Wizard. It was working
perfectly until now. For some reason when I click the button to open a form
from the switchboard, instead of showing my text and combo boxes the form
shows up blank.

I must have changed something without realizing it, if anyone can help I
would appreciate it!
