Survey: Q&A for Reps with Multi-level Combo Boxes

First of all, I am a bit of a novice at this, so seeking assistance from peer
experts. Thank you in advance for your inputs...

I have to build a form for a telephone reps Q&A survey. I have a list of
Q&As broken down by categories and questions. I would like to build a
"multi-level" combo box drop down lists to allow reps to select:
1) Category - from a drop down list of the categories; and
2) Questions/Sub-questions - from a list of questions/sub-questions
(based on the category selected by the user in (1),
this drop down list of questions/sub-questions would
be available for questions within that category)

How can I best approach this problem?
