Suggestions for Organizing Scanned Documents

We are in the process of scanning a bunch of medical documents and
wonder how we can best organize these documents as they are scanned in
the most efficient way possible.

We would basically like to scan them as PDF, OCR is not necessary, but
searchable PDF would be nice but not a requirement. We would like to
be able to keep them organized by patient, etc.

Idea we have had so far is to possibly place a label (possibly bar
code per patient) on the document with patient name and preferred file
name (ie: Smith, Joe - 070312 X-Ray), so that we can put a stack of
various docs in the scanner and have the files automatically created
w/ minimal user effort.

Any suggested past practices, software and hardware are welcome.
Currently, we are using a basic TWAIN-compliant scanner, but may be
upgrading in the future. Luckily we are just starting out and so the
number of documents is low in comparison, but we would like to get
this under control now, so we can have an efficient paperless record
system.
