start with a blank form, no data?

Im using the template above from MS and would like some help. How can i
make it so that when i click to add a new "issue", the form opens up blank
and not filled in with the first record in the database?

Also, i asked this questions yesteraday, but thought maybe i can consolidate:
Does anyone know how i can change the title on the charts in the template?

Change the Data Entry property to Yes and it will open to a new blank
record each time.

Open the 'Open Issues by Assigned to Chart Subform' in Form View (not design
view).

Right-click on the 'Issues by Assigned To' title and choose properties.
Click on the Format tab and edit the Caption at the bottom.

I tried that, but there is no option to open in form view (grayed out). I
can only do Design view, pivot chart view, and pivot table view

All I did, was double click it in the database window. Actually that opens
it in pivot chart view - so open it in that and follow the steps.

hmm...this was weird. I was trying to do this at work on Access 2002, and
did not have the option to do the change. But then i went home and tried it
