Single occurance meeting request becomes recurring

Randomly, when a meeting request is accepted it is added as a recurring
meeting instead of a single occurance. Anyone else see this or any ideas?

Is the machine on which this happens a Dell? If so, check Add or Remove
Programs to see if it have a Cyberlink Outlookaddinsetup program installed
and, if so, remove it.
