Shell/Shortcut Question RE: Start menu

What rules does the Shell follow when deciding when to add a program to the
"frequently used programs list" on the Start menu?

Several of my most frequently used programs never show up in this list,
including Visual Studio 2005, the MSDN Document Viewer, and Firefox.

Does it depend upon any properties in the shortcut used to launch the program?
Does it depend upon the file system location of the shortcut that launches
the program?

Not sure but my VS2005 is on the FUP list.
That doesn't help you though sorry.
