Search Page - password required

We had our website up and running for years. All of a sudden when using the
search page form (after clicking on "Start Search" button) we get a dialog
box requesting ID and password. Why?

I believe it has something to do with our server (we host our website
ourselves) and the website search page.
 
It sounds like the permissions are actually incorrect on the server itself.
I used to have a link to it, but can't find it, though MS does have a page
somwhere that lists the minimum NTFS requirements for the FP Server
Extensions.
