Script examples to automatically send the message form

I created a message form (VBScript - 2003) and published it. I sent the form
out to myself as testing purpose. I received a blank email. Could anyone
give me script example on how to send the form? Thanks
 
The message probably was blank because you neglected to click the Edit Read Page button in the form designer and create a read layout to match your compose layout.
