Go to Office button > Word Options > Advanced. Near the bottom, under
"Preserve fidelity when sharing this document", check the box for "Save form
data as delimited text file".

When you save the document, Word will offer to save it as Plain Text
(*.txt). If you change the suggested .txt extension to .csv, then Excel will
automatically recognize it as a comma-separated file and put the data in the
correct cells. If you leave the extension as .txt, Excel can still open it
properly, but you'll have to go through a sequence of dialogs to tell it
what to recognize.

Note that the option in Word is used for the next Save only and is not
stored as part of the Word document file, nor does it affect any other Word
documents that are open at the same time.
