Rules Wizard not processing correctly

Hey all I am running Outlook 2003 and I keep on having to create new rules
for items because Outlook is not following the rules I set. For instance I
have newsletters that I want sent to a "news" folder, so I set it up but for
some reason mail keeps on arriving in the inbox. Some of the rules run fine
others do not.

Any suggestions on why this may be happening?

How do the rules read? Do they use words in the subject / body or email
addresses?

They read the "from name" and some more advanced ones read the "subject"
