I'm looking for a form or letter outline for state represenative or other
important people to help me verify the need for disability approval with
social security office due to auto injurys and unable to walk or stand for
any length of time

I'd suggest talking to your doctor and see if he will issue the letter for
you.

ssa.gov has all the forms. It's not easy to navigate,
but they're there. Use their search engine.

Also there is a group at
