Report header - missing detail

I have created a report for our vendor payments, including name, address and balance. On some records, I am losing the data in the box I set up with city and state. It is not doing this on all records and I can find no rhyme or reason to this. I went back to my query to review the data and there is nothing missing

What could be causing these pieces to disappear. Any suggestions

What is in the fields when the problem occurs?

What is the expression you're using in the city/state text
box?
