Missing Report Header

I have created individual Inventory reports for different facilities
that we are responsible for. In the Report Header I have a sign off
sheet etc. that I want to appear as the first page on every report.
When the reports are printed called up 1 at a time the first page
prints correctly. If I use a query to call up more than 1 facility
for printing purposes the only facility that has the first sign-off
page is the first report. ALL following reports skip that page and go
straight into printing the correct inventory for that facility.

What am I missing?

I have in my Inventory Report:
Report Header (which contains the sign-off sheet)
Facility# sub report header
Location sub report header (each facility has multiple
locations with items in that location)
Item Description sub report header

A report header only prints 1 time for a report. You will need to greate a
group on facility and move the signoff sheet to the group header. You will
also want to set the Force New Page to Before Section so each facility will
start with a new page.
