Renaming .avi files to .mp4 causes explorer.exe to hang and CPU to pin at 100%

"regsvr32 /u shmedia.dll " Fixed the problem.

Simply do the following steps:
1) Open a command window (Start->Run, type in 'cmd' hit enter)
2) To remove image preview, type and hit enter
regsvr32 /u shimgvw.dll
3) To readd image preview, type and hit enter
regsvr32 shimgvw.dll
4) To remove media preview, type and hit enter
regsvr32 /u shmedia.dll
5) To readd media preview, type and hit enter
regsvr32 shmedia.dll

Why would you rename a avi file to mp4 and expect anything to work?

because the file will play in HD in RealPlayer as an mp4. It's just the CPU
goes wonky when I start to rename.

That KB article addresses a differfent issue.

Instead of simply renaming the file, you need to convert it. Look here:

What is the status of the Indexing Service?

If it is hung (stopping/starting), kill it in Task Manager.

Whatever the state, set it to manual, reboot and make sure it is not
running.

Try the rename again and report your findings.
