Remote User name on the Subject Line when submitting FORMS

I have created a very basic form which allows users to report non urgent
faults, the form does basic validation on Name, Contact Number, Location and
Fault Description fields. Once the form is completed the user would click on
Submit and the form would be sent to 1st Line Support.
The issue is that the subject line is same for every form that is submitted
i.e. if 10 different people submit the form then the subject line would be
the same on all. To make things easier I would like to change this so that
the Subject field is dynamic, possibly to read something like "Fault via
Email - Remote User name". This is assist 1st Line Support team as each fault
email will be unique with the user name.
Any ideas on how I can achieve this?

Create a field on the form called "Subject" (or whatever you'd like) and
either make it a text box that they type into or make it a drop down box of
choices you enter. Then, in Form Properties, Options. Email Results, for
Subject line click "Form field name" and enter the name of the field.

Many thanks for the response. Another question; Is there any way of achieving
this without additional field on the form?
At present the system automatically picks up the Remote User Name in the
body of the email message! Is there anyway of getting this setup for the
Subject Line?

Not with the FP Forms Handler. Without the extra field how would you
determine what the subject was?

Unless you add a field for subject and allow the users to type it in, there is no way to have a
unique subject line when using the FP Form Handler.

You could add a drop menu with subject choice the user would select from, this way subject lines
would be consistent.
