I am needing a receipt template for in-kind services for a 501(C) (3)
entity....any ideas or examples?

You will probably *never* find the exact template you need. My advice would
be to find something that has some of the elements you want and modify the
rest. (Kind of like looking for the perfect man. We both know he doesn't
exist so we find the one who meets most of our requirements and adapt.
)
