Realtek HD sometimes doesn't work

I have a Toshiba Tecra with XPSP2 and Realtek ALC861 audio device installed.
I have an intermittent problem. About 50% of the time when I start the
computer it doesn't find the Realtek audio device and I have to restart -
this usually fixes the problem but not always, and I may have to restart
again. Once there it stays and doesn't drop out. It also sometimes disappears
when the computer wakes up from hibernation.
I have installed all windows updates, including the recent patch (KB935448)
to fix the dll relocation problem.
Not a huge problem but annoying so would be glad to hear of any possible
solutions.

possibly a conflict with the onboard audio controller. you can disable
it either in bios or in device manager.

Thanks. I'm not much of a tech person. Could you tell me which of the
following I should disable - these are listed under 'Sound, video and game
controllers' in Device Manager in addition to the Realtek HDA, plus some
video controllers that I assume I don't disable:

Audio codecs
Legacy audio drivers
Media control devices

Got as far as looking .inf file, but a whole pile of them appeared. Do I
choose the one called HDA861A as I have a Realtek ALC861?
