Re-scheduled Meeting Keeps Originial One in Calendar

Hi there,
I am using Outlook 2003 with Exchange 2003.
Sometimes, when an organizer re-schedules a meeting, and I accept the
change, the new schedule shows at my Calendar Okay, but the original
schedule still keeps in the Calendar which doesn't get removed.
Any idea about this issue would be appreciated.

Any delegate actions going on? Possibly delegate and owner accepting
the update?

A canceled meeting appointment reappears in your calendar in Outlook
