Hi

I am pretty much a newbie when it comes to MS Access form design, but what I'm trying to do seems like it should be super simple, so I was hoping someone could give me a quick solution.

Basically, I have an input form where I will ask a series of yes/no questions using radio buttons. The results of these yes/no questions will then be used to generate an output document. For example, my input for might ask:

"Is the client compliant with requirement A" O Yes O No

If the answer is yes, the output document should say something like "The client is compliant with requirement A", and if the answer is no, the output document should say something like "The client is not compliant with requirement A".

I would like my input form to have a large number of these yes/no questions, and the output document to be a report on the client's compliance. Can someone tell me how to set this up in MS Access?

Thanks!
Mike
