I would like to have an Access Database to have employees answer some
questions.
I ultimately want the questions to be answered on an internal web page for
ease of distribution.

What sage advice does anyone have about doing such a thing?
I"ve created simple forms in FrontPage and such but never anything quite as
complex as this.

This may not be what you're seeking, but to save time, with not a huge investment, you might want to check:

After much consideration last year, rather than spend time on developing the solution myself, I adopted this package and don't regret the choice.

It saved me a huge slice of time and not just for the database side of the issue.

Something to consider are the esthetics and layout of offering the survey on a web page. is it a stand-alone or does it need to be integrated into an existing layout?

This software saved my bacon by formatting my surveys to fit my existing web design with complete ease, something that would have required me, a web-designer disaster, considerable time. :)

Likewise, with the survey itself, the wide variety of question types and responses are already built in, so one has virtually complete freedom of choice on question types and responses choices.

I have no association with the company beyond that of a client.
