1. The two tables must be related. Create the tables and then go to the
Relationships window to create the relationship.
2. Go to queries, click on New and choose the query wizard. Follow the
directions to build a query that includes both tables.
3. Go to forms, click on New and choose the form wizard. Follow the
directions basing the form on the query you just created.
