Publishing customized contact form

First of all I want to comment on how helpful this site, I get solution to my problems from reading other people's problem

My situation is that I have developed a second page to the contact form with user defined fields. The folder resides in the Public Folder and the customized form is published to this folder. We want to be able to pass information between the employyes (about 20 users) using this customized form....is this possible since their is default contact form in the system and will the information on the customized form stay the same for all users(Categories are defined on this customized form and want all to use these categories).

Since I am the database administrator, I really don't want all the user to have the full permission on the folder but are able to send me a new vcard using the new customized form....is there way to work around this? and is available at the click of button.

Another issue, can search be done at a click of a button rather than going through so many steps? our salespeople are not ready to go through so many clicks to search for information. Maybe this can be done writing a program, but I am not so experience at writing VBscripts.

The data always stays with the item and can be viewed on the All Fields tab
regardless of what form is used. Whether the item displays the custom form
depends on where the form is published the state of the user's local forms
